Authorization for Use of Minor’s Image and Personal Data

NOTICE: This is a legally binding agreement. Please read carefully before accepting. By checking the box, you acknowledge that you have read, understood, and accepted the terms of this agreement.

Certification of Authorization and Consent by Legal Guardian

I, the undersigned, as the parent or legal guardian of the minor named below (the “Minor”), hereby expressly authorize Sports Dream America LLC, its affiliates, partners, and/or authorized third parties to capture and use the image, voice, name, and/or any other personal data of the Minor during their participation in the activities organized by the sports camps.

 

1. Authorization for the Use of Image and Personal Data
   
   I authorize Sports Dream America LLC to record and use the image, voice, name, and/or any other personal data related to the Minor obtained during the camp activities, including online events, sports activities, exhibitions, and other related interactions, for promotional, advertising, and/or informational purposes both inside and outside the camp facilities. 
   
   
2. Specific Purposes for the Use of the Image
   
   I understand that the visual and audio content captured, including images, videos, voice recordings, and other media, may be used for the following purposes:
   
   

• Promotion of the sports camps, their activities, and related events.
• Advertising across print and digital media, including social media, websites, promotional videos, and others.
• Distribution through traditional and online media.
• Transfer to third parties for promotional and advertising purposes of Sports Dream America LLC and its business partners. 
  
  

3. Transfer of Rights to Third Parties
   
   I authorize Sports Dream America LLC to transfer the rights to the images and recordings captured during the camp to third parties, such as partners, sponsors, and collaborators, for the purposes described above. This transfer will be subject to the same terms and conditions under which the image was initially captured. 
   
   
4. Duration and Scope of the License
   
   The authorization granted is global, valid worldwide, and for an indefinite period. The images, recordings, and any content generated may be stored, reproduced, distributed, and published without limitation, in both physical and digital formats, across any medium and platform, for advertising or promotional purposes. 
   
   
5. Waiver of Compensation
   
   I acknowledge and agree that the use of the Minor’s image will be free of charge, and no monetary compensation or payment will be made for the use of said image or recordings. I further waive any rights to receive remuneration, payment, or compensation now or in the future for the use of the materials captured. 
   
   
6. Right to Withhold Image Use
   
   I understand that I have the right to withhold authorization for the use of the Minor’s image and data. If I do not agree with the terms outlined herein, I may notify Sports Dream America LLC by sending an email to info@carlosalcarazcamps.com, indicating my wish for the Minor’s images to not be used. Such notification must be made prior to the start date of the activities. 
   
   
7. Compliance with the Children’s Online Privacy Protection Act (COPPA)
   
   In accordance with the Children’s Online Privacy Protection Act (COPPA) of the United States, I confirm that I have provided the necessary consent for the collection and use of the Minor’s personal information in relation to the camp activities. I understand that Sports Dream America LLC complies with this law and takes reasonable measures to protect the Minor’s privacy and safety during their participation in the activities. 
   
   
8. Protection of Personal Data
   
   I confirm that the personal data provided will be treated confidentially and will only be used for the purposes described above. The Minor’s personal data will be stored securely and processed in accordance with applicable data protection laws in the United States and in compliance with COPPA. The parent or legal guardian of the Minor may exercise their rights of access, correction, cancellation, opposition, limitation, or portability of the data by sending a request to the email address provided. 
   
   
9. Indemnification
   
   I understand that, if any third party to whom Sports Dream America LLC has transferred the images or recordings of the Minor violates the terms of this authorization, such third party will be responsible for any claims, damages, or expenses arising from any negligence or misuse that may violate the Minor’s integrity, dignity, honor, and image. I hold Sports Dream America LLC harmless from any liability in the event of inappropriate use by a third party outside of its control. 
   
   
10. Confirmation of Consent
    
    By proceeding with the purchase or registration for the sports camp, it will be considered that I have given my explicit consent for the use of the Minor’s image and data in accordance with the terms of this document. If I do not agree, I commit to notifying my opposition by email prior to the Minor’s participation in the activities. 

    Applicable Laws:
    
    This document complies with applicable privacy and data protection laws in the United States, including the Children’s Online Privacy Protection Act (COPPA), as well as applicable state privacy laws where relevant. Sports Dream America LLC is committed to adhering to data protection regulations and ensuring the privacy and security of the Minor during their participation in the activities.
